Suicide Protocol and Medical Re-Entry Protocol

The College recognizes that a student’s physical, behavioral, and emotional health is an integral component of a student’s academic success at the College. The purpose of the protocol is to protect the health and well-being of all students by having procedures in place to identify, assess the risk of, intervene in, and respond to suicidal behavior. The policy is activated when the College has actual knowledge that a student is actively engaged in suicidal behavior, has previously engaged in suicidal behavior while enrolled at the College or recently before matriculation, or has stated plans or intentions to commit suicide. A student who engages in any of these behaviors may be required to comply with the College’s Re-Entry Policy before being permitted to resume classes. 

The Student Suicide Prevention Protocol is administered by the Director of Community Standards and copies of the protocol or the College’s Medical Re-Entry Policy are available in the Dean of Academic and Student Development's office or electronically upon request.  Please contact the Director of Community Standards at 978-762-4000 ext. 2197 for more information.

Medical Re-Entry to North Shore Community College:

Any student who is hospitalized while enrolled at North Shore Community College must comply with the North Shore’s re-entry process before resuming classes. This process is intended to ensure that a student who is returning from a hospitalization is capable of successfully resuming their academic activities. Students who are absent for more than two years must also Apply for Readmission. 

The process is as follows:

  1. Student must complete and submit the NSCC Student Medical Re-Entry Form.
  2. Student’s licensed healthcare provider completes and submits a Health Care Provider Medical Re-Entry Form.
  3. Forms should be delivered as indicated. 
  4. Following the submission of both forms, the student shall engage in a re-entry interview to determine if and when it is appropriate for the student to resume their studies.

The Student Medical Re-entry Form‌, the Health Care Provider Medical Re-Entry Form, and any related clinical documentation will be shared only with appropriate professional staff members at North Shore Community College, will be kept confidential, and will not be connected to the student’s enrollment records. It may also be necessary for a student to provide a release of confidentiality to allow professional staff at North Shore Community College to speak with the student’s medical or mental health provider(s).
